Accelerating value-based care

Through the combined capabilities of UnitedHealthcare and Optum, we are accelerating the transition from a fee-for-service to a value-based system of care delivery. Value-based care arrangements are designed to manage health care costs and improve the patient experience.

For patients:
We are simplifying the health care experience and focusing on whole-person health, making high-quality care accessible whenever and wherever patients need it, and enabling better outcomes.
For care providers:
We are creating greater capacity for patient care, ensuring physicians have the time, tools and insights they need to not only care for patients when they are sick, but to help keep them well.
For payers:
We’re helping every health care dollar go further by making high-quality care more accessible and affordable, and making it easier to keep chronic conditions in check.

Collaboration with Walmart

Through our collaboration with Walmart, clinicians at Walmart Health centers will have access to Optum’s analytics and decision support tools to help them deliver comprehensive value-based care to Medicare beneficiaries. Additionally, going forward we will address social determinants of health by improving access to benefits such as healthy food, medications, dental and vision services.

Ultimately this collaboration will serve hundreds of thousands of Medicare beneficiaries in value-based arrangements through multiple Medicare Advantage plans.

Value-based care at home

Thousands of Optum clinicians are bridging the home and clinic, bringing together nurse practitioners, social workers, pharmacists and other experts to coordinate care for the growing number of patients who require more individualized care. Many have disabilities, struggle with mental health issues, lack transportation and live in remote areas. On average, these patients are managing nine chronic conditions and multiple medications. 

Optum care teams serve as an extension of the patient’s primary care physician, building relationships and earning trust during in-home visits. This integrated approach leads to better outcomes, including a 12% reduction in hospitalizations.
